Nexus Primary Health supports people to fully participate within their community through the provision of appropriate wide-ranging support services throughout their life span. With a 40-year history, we are the "go-to" organisation for primary health information and person-centred services in the outer and rural north of Melbourne.
We are currently recruiting for Community Support Workers who can support members of the local community to remain living safely in the comfort of their own homes.
Let's talk about the role
Community Support Workers provide whole of client support in order to empower clients with disabilities and frail aged clients to lead fulfilling lives according to their own wishes. Providing quality, practical, efficient, and effective services to clients include but is not limited to personal care assistance, respite care, and domestic assistance.
Let's talk about you
You have a strong commitment to working from an empowerment perspective, promoting the client's independence, skill development, and self-advocacy.
You have a Certificate III in Home & Community Care, Aged Care, Disability or Individual Support.
You must be available for a minimum 25-hours per week, which may include early mornings, late afternoons, and some weekends. Penalty rates apply as per the Award.
